Just by way of comparison, what does everyone consider a standard prematch warm-up?

Obviously it depends on level. Doing an adult game at around L7-8, I'll generally do the prematch talks about 30 minutes out, then:
- jog around the pitch (checking the touchlines and in-goal for any unwanted objects... having found concrete blocks and dog poo in the past)
- use a 20x10m square to do 5 circuits of sprints, side-skips and reverse running
- stretch out hams and hips, sit in a low squat for a while, and jog it off
- practice any running lines I want to work on for the match ahead
 
I jog round the pitch and in passing I give the posts a shake (being careful not to have the cross bar fall on my head)
 
I do a fairly basic set of stretches and jogging similar to @Rich_NL (I get a bit lazy on the sprints, and don't practice my running lines...).

BUT I'm more serious about my warm down than my warm up - if I don't warm down I tend to get sore and stiff in the following 48hrs.

Mentally, I have a printed copy of my 'work ons' and I re-read before the game to remind myself.
 
- lap jog, same as yours that doubles as a pitch inspection
- alternating sprint/jog across the width (sprint to 5, jog to 15, sprint to other 15, etc)
- karaokes, stepovers, hamstring/calf stretches

what do you do to waken yourself mentally?

For me, if I have ample time and am not disrupting a team's warm-up, I'll grab a ball and knock over some drop goals from around the 22m, jogging to the ball between kicks.

That's a great question! I usually do that before leaving the changing room. 5-10 minutes of yoga nidra/NSDR, and then I think about my work-ons for the game and jot them at the bottom of my scoresheet.
 
I always worry about refs who do a 40 minute warm up; what about stamina for the last ten minutes of the game?

I understand if you "need to warm up" but don't forget how much this can affect your 'fitness' late on.

Work on your running lines and positioning -it will keep you refereeing for years longer than you might think. Work smart, not work harder. I FULLY UNDERSTAND THIS IS DIFFERENT FOR REFS AT DIFFERENT LEVEL.

Please do what works for you - add a bit, subtract a bit and see what actual difference it actually makes TO YOU.

Different level have different standards, different refs also have different standards of achievement and enjoyment.

When I was in the States, I had a play list for the 15 minutes prior to kick off.

Over here, have a bit of a yarn with the ARs as we wait to find out if we have a game, and then flip a mental switch if we do.
