A player or player actions caused the ball to become dead. Our whistle merly says we have decided that to be the case.

A player kicks the ball off the field into the crowd. We blow to say the ball is dead. We did not make it so the player did.

If they just swap those last 2 columns and then draw the table in a way that those cells are blank and not part of the table - that should do it.
